  6.                 TESTAMENT

  8.     If the time comes when I am incapacitated to the 
  9. point where I can no longer actively take part in decisions 
  10. for my own life, and am unable to direct my physician as to 
  11. my own medical care, I wish this statement to stand as a 
  12. testament of my wishes. I Janice Rand request that I be allowed to
  13. die 
  14. and not be kept alive through life support system if my condition 
  15. is deemed terminal. I do not intend any direct taking of my life, 
  16. but only that my dying not be unreasonably prolonged.
